What to Expect

  • Our post-hospital rehab team creates individualized plans for each client, assuring that every single person gets the level of care that best meets their needs while in post-hospital rehab.
  • At the center of the post-hospital rehab team are the client and family. Staff team members include a physician, nurse, physical, occupational and speech therapists, social workers and a dietitian, who all work together to develop a plan of action to help the client return home as soon as possible.
  • Post-hospital rehab therapists work with clients on the individual skills they use during daily life tailoring therapy for their specific needs.
  • Therapists use a real-life rehab apartment complete with assistive products from an on-site shoppe
(both detailed below), a car and other familiar locations to practice those skills safely for greater peace of mind for the return back to life at home.

To ease what can be a stressful situation, we take reservations for planned surgeries; offer 24/7 admission, and invite hospital physicians to follow their patients while at Menorah Park's Marcus Post-Hospital Rehab Center. Read how we effectively support care continuity with community physicians and the added support of physician assistants.

The same team also offers comprehensive discharge planning and support services for home.

An extra level of care is supported by physician assistants in addition to two full-time physicians, registered nurses, a dedicated social worker and activities professional, on-site therapy specialists, and highly trained nursing assistants.

Post-Hospital Land-Based and Aquatic Physical Therapy, Occupational and Speech Therapies

The Menorah Park Outpatient Rehabilitation Services staff is a strong member of Menorah Park Marcus physical therapy; post-hospital therapyPost-Hospital Rehabilitation coordinated care team. These health care professionals engage in ongoing training and are knowledgeable in the latest techniques in occupational, aquatic and land-based physical therapy that facilitate healing, encourage prevention, and instill lifelong wellness for adults of all ages.

They are specialists in care after knee, hip, back and other surgeries, as well as recovery from a stroke, or a cardiac condition.

As geriatric experts, they also effectively support pain reduction and strengthening after illness with targeted occupational and physical therapy designed for aging adults.

When faced with aches, pains or reduced mobility, the one thing that every client wants is quick, convenient access to solutions that are both specific to their needs and include results that help improve overall well-being.

We believe as do our many clients that one-size-fits-all therapy does not properly address the unique needs of the senior population. Our physical and occupational therapists understand and address age-related muscle loss and other physical changes as they work to support your desire to return home safely and independently. Therapy staff are critical members of the transitional care team.
