Inpatient post-hospital, or subacute care at Menorah Park's Marcus Short-Term Rehabilitation Center provides clients with high quality care as they recover from an illness, injury or surgery. The subacute health care team consists of a social worker, physician, nurses and rehabilitation therapists who work together to enable clients to return to their own homes. Therapies provided include occupational, physical, speech, aquatic and music, depending on the client’s needs. The newest addition to the Marcus Short-Term Rehabilitation Center
is the Real-Life Rehab Suite where clients can: *Practice skills safely with one-to-one guidance of a therapist *Find realistic opportunities for clients and family to experience
and adapt to changes *Identify safety riskes and address them before returning home *Try adaptations and tools that can be used at home for
greater independence As part of the discharge planning process, the care team coordinates
a package of services as needed – from home health aides and housekeepers
to adaptive equipment. As clients return to daily life, staff can also
coordinate visits to our extensive outpatient rehabilitation program found
at the Peter B. Lewis Aquatic & Therapy Center.
