The Menorah Park Social Work Department provides a wide range of supportive services to residents and their families. They are compassionate and respectful as they assist with the application process, the transition into Menorah Park and day-to-day life issues. Each pavilion has a social worker who:
- serves as an advocate for residents and families
- coordinates intake and admissions
- counsels – individual, family and group
- coordinates conferences and family meetings
- assists in accessing community resources
- provides assistance with financial matters – Medicaid applications and resident banking
- educates residents, family and staff
- coordinats short-term, post-hospital rehabilitation
- participates in interdisciplinary team approach to resident care
- assesses and promoting resident quality-of-life issues
- assists in the purchase of clothing and assistive devices
- provides aging and bereavement counseling
Our Social Workers are always ready to discuss your needs and help you evaluate whether you or your loved one should start the admissions process. Additionally, they can offer you leads to resources that may help ease the waiting period before admission or even avoid nursing home admission when other choices are more appropriate.
The Application Process
An application can be filled out online, by mail or fax, or by making an appointment with one of our Social Workers. At Menorah Park, we know applying to a nursing home can be a difficult time in a family's life, and we are committed to easing the process as much as we can.

Submitting an Application
Online – Click here to fill out the online application. You need Acrobat Reader version 3 or above – download the free Acrobat Reader here. This application is processed by a secure server and is safe even for submission of personal and financial information. Only authorized personnel will be able to see your application during this process. When you have finished filling out the form as best you can, click on the submit button at the bottom of page three. Within a few working days, someone from the Social Work Department will contact you.

Fax – Print the application, fill it out and fax it to 216-831-5266.
The Follow Up
Once the application is complete, a meeting will be arranged with a Social Worker, the applicant and family to discuss and evaluate the applicant's current and projected care needs. Once approved for admission, the client will be placed on a waiting list for the next available opening. Waiting times vary depending on what level and type of care is needed. Your Social Worker will give you a projection and will offer you referrals to other resources as appropriate to ease your time during the waiting period.
